Authorities are investigating a fatal multi-vehicle crash that occurred at the intersection of Lebanon Church Road and Buttermilk Hollow Road in West Mifflin, where multiple emergency agencies responded to a serious collision involving three vehicles.

Emergency dispatchers received reports of the crash near the area of West Mifflin Volunteer Fire Company No. 3, prompting an immediate response from firefighters, police officers, and emergency medical personnel. Initial reports from the scene indicated that three vehicles were involved in the collision, with at least one vehicle overturning as a result of the impact. Responders also reported that one of the occupants was trapped inside a vehicle, requiring a technical rescue before medical personnel could reach the victim.

Incident commanders quickly requested that Lebanon Church Road be closed in both directions around the crash site to allow emergency crews to safely conduct rescue operations and begin their investigation. Motorists were urged to avoid the area as traffic was diverted around the scene, leading to significant delays during the emergency response.

Authorities later confirmed that one person died as a result of the crash. Officials have not yet released the identity of the deceased, pending notification of the victim’s next of kin. No additional information regarding the age or gender of the victim has been made public.

At this time, officials have also not confirmed the total number of people injured in the collision or the severity of those injuries. Several occupants were evaluated by emergency medical personnel at the scene, and additional patients may have been transported to nearby hospitals for treatment. Authorities are expected to provide updated information as the investigation progresses.

The exact sequence of events leading to the crash remains under investigation. Police are working to determine what factors may have contributed to the collision, including whether speed, driver impairment, distracted driving, roadway conditions, or mechanical issues played any role. Because of the complexity of the incident, the roadway remained closed for an extended period while investigators conducted measurements, photographed the scene, and cleared debris from the roadway. Tow trucks were also called in to remove the heavily damaged vehicles, including the overturned vehicle.

Serious crashes at major intersections often require extensive investigations, particularly when a fatality is involved. Officials are asking anyone who witnessed the collision or who may have dash camera footage from the area around the time of the crash to contact investigators.
